In relation to this, the Bayambang Water District Water Safety Plan (WSP) is now certified and approved in compliance with the requirements in pursuant to the Department of Health (DOH) Administrative Order No. 2017-0006 entitled Guidelines for the Review and Approval of the Water Safety Plans of Drinking Water Service Providers. The WSP Acceptance Certificate Control No. 2022-003WD was issued last September 26, 2022 and valid until September 25, 2025. Bayambang Water District, under the leadership of GM Francis J. The SOA will be issued immediately to every concessionnaire after the automatic meter reading device computers and analyzes status data from your water meter based on real-time consumption. The Bayambang Water District gladly announce that starting this week of September 2018, we will be issuing our new Statement of Account(SOA) Form as part of the upgrade of our Billing and Collection System. The SOA will be issued immediately to every concessionnaire after the automatic meter reading device computers and analyzes status data from your water meter based on real-time consumption.

Herewith are examples of the new SOA form. Notice for Disconnection will also be indicated below after two (2) months of application for new water connection office during payment.
